  3. If you have Aetna, United Healthcare, or Cigna, after completing the form above, you must also click HERE to book your first appointment. Headway will be used to verify your insurance and for billing. 


For private pay, a superbill can be generated at your request, which you can use to submit to your insurance company for reimbursement purposes.
